To request medical records please contact the Keeper of the Records at the main office line. (508) 991-6390.
Please be prepared to provide the following information:
- Date of service
- Patient Name
- Patient Date of birth
Billing Information
Ambulance Billing is handled by COMSTAR Ambulance Billing. Any questions should be directed there.
