Please review enclosed paperwork that is required to file/renew your Business Certificate.

Please call the Office of the City Clerk with any questions at 508-979-1450.

After forms are completed and notarized, please mail a check for $40.00 payable to the City of New Bedford with the original forms.

  1. Business Name / Address / Business Telephone Number
  2. Full Name of Owner(s) / Home Phone / Home Address
  3. Signatures of Owners
  4. Notary Information

Workmen’s Compensation Affidavit

  1. Applicant information should be printed or typed
  2. If NO employees, check the box “I am sole proprietor and have no one working in any capacity”
  3. Sign / Date / Print name and home telephone number
  4. If there is Workmen’s Compensation Insurance, fill out appropriate information and include a copy of policy

Discontinuance of Business Certificate

  1. Verify information
  2. Signatures of Owners
  3. Notary Information

After Discontinuance form is completed and notarized, please mail a check for $20.00 payable to the City of New Bedford with original form.
