Please review enclosed paperwork that is required to file/renew your Business Certificate.
Please call the Office of the City Clerk with any questions at 508-979-1450.
After forms are completed and notarized, please mail a check for $40.00 payable to the City of New Bedford with the original forms.

- Full Name of Owner(s) / Home Phone / Home Address
- Signatures of Owners
- Notary Information
Workmen’s Compensation Affidavit
- Applicant information should be printed or typed
- If NO employees, check the box “I am sole proprietor and have no one working in any capacity”
- Sign / Date / Print name and home telephone number
- If there is Workmen’s Compensation Insurance, fill out appropriate information and include a copy of policy
